  • Extensive experience with iStent Inject, Hydrus, OMNI, GATT, and Kahook Dual-Blade

Am I a Candidate for Glaucoma?

You can have glaucoma and feel completely fine, which is why screening matters. If any of these apply to you, schedule an exam with our glaucoma specialists.
No Symptoms at All
Most glaucoma is painless and symptom-free until peripheral vision is already affected.
Family History
A parent or sibling with glaucoma raises your own risk of developing it.
Sudden Eye Pain or Halos
Sudden pain, headache, or halos around lights can signal angle-closure glaucoma, an emergency.
Overdue for Screening
Adults over 40, and anyone with diabetes or high eye pressure, should be screened regularly.

A note on urgency: angle-closure glaucoma can develop rapidly and cause eye pain, headache, or blurred vision. This is a medical emergency and should be evaluated immediately.

Family history matters: because there can be a genetic component to glaucoma, close relatives of a diagnosed patient should be screened as well.

What Is Glaucoma?

Glaucoma is an eye disease that damages the optic nerve, the cable that carries visual information from the eye to the brain, often associated with elevated pressure inside the eye. Primary open-angle glaucoma (POAG) is the most common form. Its exact cause isn’t fully understood, and there is currently no cure, though progression can be slowed or stopped with treatment.

Because glaucoma damages peripheral vision first, most patients don’t notice a problem until an eye doctor detects it during an exam. A comprehensive glaucoma evaluation at Sierra Nevada Eye Center includes measurement of eye pressure, corneal thickness, gonioscopy to examine the eye’s drainage structures, and optical coherence tomography (OCT) to assess the optic nerve itself.

Treatment starts with the least invasive option that will control eye pressure. For many patients, that’s a daily eye drop or a quick in-office laser procedure such as selective laser trabeculoplasty (SLT) or direct selective laser trabeculoplasty (DSLT). When these aren’t enough, minimally invasive glaucoma surgery (MIGS) offers a lower-risk alternative to traditional glaucoma surgery. Dr. Wolff was the first surgeon in Northern Nevada to implant the iStent, and our surgeons have some of the most extensive MIGS experience in the state, including iStent Inject, Hydrus microstent, OMNI, GATT, and Kahook Dual-Blade. Many of these devices can be placed at the same time as cataract surgery.

Other laser procedures treat specific types of glaucoma. Laser iridotomy creates a small opening in the iris to give fluid an alternate pathway, and is used for narrow-angle and angle-closure glaucoma. It is a quick procedure performed in the office or an outpatient surgery center. Endocyclophotocoagulation (ECP) treats the ciliary body, the tissue that produces fluid in the eye, to lower fluid production, and is often combined with cataract surgery. Transscleral cyclophotocoagulation (CPC) applies laser energy through the white wall of the eye to reduce fluid production and lower eye pressure. ECP and CPC are performed in an outpatient surgery center.

For glaucoma that doesn’t respond to drops, laser, or MIGS, our surgeons also have experience with traditional glaucoma surgeries, performed when a more significant reduction in eye pressure is needed. These include trabeculectomy, which creates an opening that lets fluid drain out of the eye, and tube shunts, including the ExPress mini-shunt and the Ahmed and Baerveldt shunts. Canaloplasty, an FDA-approved procedure, widens the eye’s natural drainage canal to improve outflow while avoiding some of the risks of other glaucoma surgeries.

Comparing Glaucoma Treatment Approaches

Glaucoma treatment isn't one-size-fits-all. Our specialists tailor a plan based on the type and severity of your glaucoma.

Medication & Laser (SLT)

Lowers eye pressure with drops or a quick in-office laser

  • Eye drops reduce fluid production or increase drainage
  • Selective laser trabeculoplasty (SLT) is quick and non-invasive
  • Often the starting point for newly diagnosed glaucoma
  • Requires consistent daily use and follow-up monitoring
  • No incisions involved
First-Line Treatment

Minimally Invasive Glaucoma Surgery (MIGS)

Fewer complications than traditional glaucoma surgery

  • iStent Inject can be placed during cataract surgery
  • Also includes Hydrus microstent, OMNI, GATT, and Kahook Dual-Blade
  • Designed to reduce or eliminate the need for daily drops
  • Fewer risks than traditional trabeculectomy or tube shunts
  • Our surgeons have some of the most extensive MIGS experience in Nevada
Advanced Option

Direct SLT (DSLT)

Automated laser treatment for the eye's drainage system

  • Non-invasive and automated, delivered in seconds
  • No incisions, minimal discomfort, and no recovery time
  • Treats the eye's natural drainage system to lower eye pressure
  • May reduce or eliminate the need for daily eye drops
  • Can be used as an early treatment or alongside other glaucoma care
Automated Laser Option

Ask your doctor at Sierra Nevada Eye Center whether medication, laser (SLT or DSLT), or MIGS is the right starting point for your glaucoma.

Your Glaucoma Care Journey

Ongoing care to protect your vision for the long term.
1
Diagnosis
  • Eye pressure measurement and gonioscopy
  • OCT imaging and visual field testing
  • Family history review
2
Treatment Selected
    • Eye drops or SLT laser started for many patients
    • MIGS considered for select candidates
    • Traditional surgery reserved for advanced cases
3
Follow-Up
    • Pressure rechecked within weeks of starting treatment
    • Medication or procedure adjusted as needed
    • Side effects and drop adherence reviewed
4
Long-Term Monitoring
    • Lifelong monitoring to catch changes early
    • Regular OCT and visual field testing
    • Family screening recommended where appropriate

Expected Glaucoma Outcomes & Recovery

Glaucoma treatment is about protecting the vision you have. Here's what patients can generally expect.
Preserved Vision
The goal of every treatment is to protect the vision you have today from further loss.
Reduced Drop Burden
MIGS can reduce or eliminate the need for daily glaucoma medication for many patients.
Fast Recovery
Laser and MIGS procedures are typically outpatient with a short recovery period.
Lifelong Monitoring
Regular follow-up ensures any changes in eye pressure or optic nerve health are caught early.

What Patients Can Expect in the First 30 Days

  • Mild irritation after SLT laser, resolving within a day or two
  • Eye pressure rechecked four to six weeks after starting a new treatment
  • Some MIGS patients reduce or eliminate daily eye drops
  • Vision loss that has already occurred cannot be reversed
  • Ongoing monitoring continues indefinitely, even after successful treatment
  • Combined cataract and MIGS patients recover on the same timeline as cataract surgery alone

Safety & Risk Considerations

Glaucoma treatments range from a daily eye drop to same-day surgery, and each carries a different, generally well-understood safety profile.

Safety Profile

  • SLT laser is non-invasive with minimal downtime
  • MIGS carries fewer complications than traditional glaucoma surgery
  • Most glaucoma procedures are performed under local anesthesia
  • Traditional surgery is reserved for cases that don't respond to less invasive options

Possible Side Effects

  • Temporary Eye Irritation - common after laser or MIGS, usually resolving within days.
  • Fluctuating Eye Pressure - occasionally requires a medication adjustment after a procedure.
  • Reduced Effect Over Time - some procedures may need to be repeated or supplemented with medication years later.
